The circulating nurse removes the sealed pouch containing the rightMEDlabels.

The U.S. Pharmacopoeia (USP) determined that in a four-year period, 20.6% of operating room medication errors occurred because of unauthorized or wrong drug use, with many of the events resulting in harm to the patient. Soon after, USP gave the prescription to improve the medication labeling.

The labels are attached to original medication vials and other containers in a sterilized peel pouch for immediate use in the sterile field.
